Causes and manifestations of this disorder

Vitiligo is a skin disorder that causes loss of skin color. Smooth white areas (macules, spots) appear on the human skin. It generally starts on the hands, forearms, feet and face. This condition occurs when melanocytes (skin cells that produce melanin, which gives the skin its color or pigmentation) are destroyed by the immune system.

Vitiligo affects people of all skin types, but it can be more pronounced in people with darker skin. This condition is not life-threatening or contagious. Worldwide, about 1% of the population has vitiligo.

Although the causes of vitiligo are not fully understood, there are a number of different theories:

  • autoimmune disorder: the affected person’s immune system may develop antibodies that destroy melanocytes
  • genetic factors: about 30% of vitiligo cases are observed in families,
  • genetic factors: about 30% of vitiligo cases run in families, neurogenic factors: a substance that is toxic to melanocytes can be released on nerve endings in the skin
  • self-destruction: a defect in melanocytes causes them to destroy themselves.

Vitiligo is also associated with other autoimmune conditions, such as hyperthyroidism (an overactive thyroid gland), but not everyone with vitiligo develops these conditions. Vitiligo can also be triggered by certain events, such as physical or emotional stress. It is also possible that a combination of these factors is responsible for vitiligo.

Symptoms of vitiligo include:

  • slight loss of skin color, which usually first appears on the hands, face, and areas around body openings and genitals
  • premature fading or graying of hair on the scalp, eyelashes, eyebrows or moustache
  • loss of color on mucous membranes

Vitiligo can start at any age, but usually appears before the age of 30. Vitiligo can be:

  • generalized – the most common type, when spots appear on different parts of the body
  • segmental – limited to one side of the body or one area, such as the hands or face
  • mucosal – affects the mucous membranes of the mouth and/or genitals
  • focal – a rare type in which the macules are in a small area and do not spread in a particular pattern within one to two years
  • trichome – represents a white or colorless center, then an area of lighter pigmentation, then an area of normally colored skin
  • universal – another rare type of vitiligo in which more than 80% of the body’s skin lacks pigment.
